Immobile residents load the same tissue hour after hour — sacrum, trochanters, heels — and a Stage 1 finding can progress before the next repositioning round is charted. Alternating pressure redistribution is the support-surface answer: rather than holding the body on one static plane, the surface itself cycles, so no bony prominence carries sustained interface pressure. For skilled nursing, long-term care, rehab, hospice, and home health fleets, these systems are a core line item in HAPI prevention programs — supporting care-plan documentation, easing the burden on turn schedules, and standing up to survey scrutiny.

Each system pairs a variable-pressure pump with a mattress or overlay built from individually replaceable air cells — typically 17 to 21 cells, depending on the platform. On a fixed cycle, commonly 10 to 25 minutes, the pump inflates alternating cells while the opposing set deflates, restoring capillary perfusion to off-loaded tissue before the next phase begins. Standard features across the category include weight-based pressure calibration, a static/float mode for transfers, wound care, and meals, a CPR quick-dump valve, and audible low-pressure and power-failure alarms. Covers are waterproof, vapor-permeable, low-shear, and rated for hospital-grade disinfectants; low air loss models add microvented airflow through the cell surface for microclimate control. Standard capacities run 300–450 lb, with bariatric platforms rated to 1,000 lb, and most systems fit a 36" x 80" hospital deck.

Selection Tip: Specify by risk stratification first, then by frame. Overlays (3"–5") sit on an existing mattress and suit Braden scores in the at-risk to moderate range or Stage 1–2 prevention protocols; full replacement systems (8"–10") are indicated for high-risk residents, existing Stage 3–4 wounds, and anyone essentially bed-bound. Add low air loss when moisture or maceration is a documented factor. Confirm deck dimensions and side-rail clearance before ordering, and spec weight capacity with margin above the resident's actual weight.
